Initial Comments:
Based on the findings of an offsite follow-up survey conducted o March 10, 2022, Solidarity Home Healthcare Services, Llc., correct deficiencies previously cited under the requirements of 28 Pa. Code, Health Facilities, Part IV, Chapter 611, Subpart H. Home Care Agencies and Home Care Registries. The deficiencies were a result of an onsite State Re-Licensure survey conducted on May 11, 2021. The Agency failed to follow its Plan of Correction dated 5/31/21 resulting in 2 deficiencies that remain out of compliance and re-cited as part of an on-site follow-up survey conducted on .October 25, 2021
